Invoice Payment

Complete this registration form if you are part of a group whose registration fees will be paid by invoice. Each attendee in the group should complete this form to ensure that they receive event emails. Please work with your colleagues and the business office to ensure that the same billing office email address is listed for all attendees in your group. Invoices must be paid to receive a certificate of attendance or ASHA CEUs for the event.
